The fee is $195 per session. This practice accepts major credit cards and HSA/FSA cards. Clients are responsible for contacting their insurance provider regarding out-of-network benefits. Reimbursement is not guaranteed.

I understand that I may be provided with referrals if my needs fall outside the scope of this practice. If my appointment is confirmed, I agree to complete and sign all required forms prior to the first session, including practice policies, consent forms, and intake questionnaires.
